Be Prepared for Your First Visit

  • Please arrive 15 minutes early on the day of your appointment to fill out the paperwork.
  • You may also print the New Patient Form and bring the completed form on the day of your appointment.
  • Please bring your current insurance card(s) if you would like to file for insurance and a list of your current medications.
  • When scheduling your appointment, please indicate if your visit is related to a motor vehicle accident or work injury. Also, please bring your auto insurance or worker's compensation information, including claim numbers, billing addresses, and phone numbers.
